By submitting this form, I grant to the Foundation and its assignees and licensees a perpetual, royalty-free worldwide license to use, copy, share, modify, publicly display, publicly perform, broadcast and distribute in any format, including without limitation via the Internet, intranets, CD-ROM and other technologies, and for any purpose any information I provide to the Foundation about myself and my family, including my/my family’s name, voice, likeness, photograph, and any additional information I have provided to the Foundation. I represent to the Foundation that I have gotten all permissions necessary to share and license photographs and other provided information from any necessary third parties.
